Referral Deals collects contact details and request information so a referral request or provider signup can be reviewed and followed up on. The proof-of-concept does not publish provider profiles or public customer requests.

Information submitted through the forms may be emailed to the site operator and stored in submission logs on the hosting account. Do not submit sensitive personal, medical, financial, or legal documents through the form.
